Outpatient & Outreach Services
The Acquired Brain Injury Program outpatient services:
- Outpatient Team
- clients are referred from the inpatient ABI program, acute care or
from service providers in the community
- clinicians collaborate with clients and their families to achieve
successful community integration
- intervention is goal based and time limited within the focus of attaining
an optimal level of functioning in the community
- The Inpatient, Outpatient and Outreach services of the Acquired Brain
Injury Program all function as an interdisciplinary team in providing
rehabilitation services.
- Outreach Services
- provincial resource
- provides help to the community in finding the best available
resources to support clients. This may be achieved through goal
setting; assessment; identification of resources available in
the client's community; recommendations to achieve goals in the
community; and advocacy for obtaining available resources
- clients are primarily assessed and followed-up in their own
community.
- Self-Management Program
- for clients with chronic mild brain injury symptoms
- clients work with an occupational therapist and a psychologist
for a 10 week period
- it is a partnership in that all action plans are jointly devised
and clients are expected to be accountable for taking steps to meet
their functional life goals.
